Our own Dr. George Hariz will retire after years of service to the community. Below is a letter from Dr. Hariz to his patients:
Dear valued patients,
It is with mixed emotions that I announce my retirement from Dallas Medical Physician Group. My last day of practice will be January 6, 2025. It has been a great pleasure providing your healthcare needs over these many years.
Your medical records will remain with Dallas Medical Physician Group and will be available for your continued care. Your records are kept confidential, and a copy can be released to you for a copying fee or transferred to another physician of your choice without a fee. To obtain a copy of your medical records, please call our office at 972-270-7500 to request a medical records release form.
We have other providers in Dallas Medical Physician Group, and we are happy to schedule a visit for you. You may also contact your insurance company for a list of providers in your area, or the local medical society can suggest other physicians in your area who are accepting new patients.
I have greatly valued our relationship and thank you for your loyalty and friendship over the years.
